Health and Wellbeing Survey

Thank you for participating in the WA Primary Health Alliance Health and Wellbeing survey, designed to get a ‘baseline’ picture of the general health and wellbeing of our employees. Gathering this information now will help us to understand what difference the implementation of the Health and Wellbeing Framework will make to our employees.

The results will be used to:

  • Identify specific challenges staff are facing from a health & wellbeing perspective in a safe, secure & anonymous way.
  • Serve as a tool to measure staff awareness levels, attitudes and behaviours regarding health & wellbeing both before & post the health & wellbeing framework rollout
  • Identify barriers in the workplace that are preventing staff from engaging in healthy behaviours
  • Identify appropriate initiatives & programs that will support and improve staff health & wellbeing
  • Serve as a tool to measure impact & opportunities to review/improve the health & wellbeing framework as the program develops.

All the questions are voluntary and only the health & wellbeing working group will have access to the survey results. The results will be used in conjunction with the information gathered in the H&WB focus group sessions to help identify key themes for the health & wellbeing framework. We will not publicise any personal information.
