Wheatbelt Chronic Conditions Network will hold their first meeting for 2018: February in Northam
In October, WACHS Wheatbelt Population Health Manager, Chantelle Jeffery and Chronic Conditions Coordinator, Karen Beardsmore met with WAPHA to plan the 2018 Wheatbelt Chronic Conditions Network meetings. It was decided that we would convene four meetings a year; one in each hub town, with VC to other sites.
The purpose of the meetings is to provide opportunity to network, whilst sharing information and innovation in patient centred care, with a focus on self management of chronic conditions.
The meetings are intended to be educational, not a replacement for professional development, and open to public and private sector chronic conditions clinicians and care coordinators. It is proposed that the host site will be responsible for producing a post meeting summary (not minutes), in the form of a one page newsletter, to be uploaded to this site.
