As part of WA Primary Health Alliance (WAPHA) Commonwealth reporting requirements, we are updating our records regarding accreditation status for WAPHA-funded mental health services. The Clinical Governance Survey below includes questions for your WAPHA-funded mental health service(s) that undertake clinical intervention, and will take approximately 5 minutes to complete. Please complete the survey below by Friday 19 March.

A clinical intervention is that which is undertaken (or supervised) by a (registered) clinician (including allied health) within their defined scope of practice.

Accreditation means that the organisation has demonstrated, through an independent review and assessment, that they have implemented the standards. A certificate or accreditation award is provided to the organisation if accreditation is achieved.

Compliance indicates that your organisation has undertaken a self-assessment, including gathering evidence, implementing actions and monitoring progress against the standards. There are resources and tools available from the Australian Commission on Safety and Quality in Health Care to support a self-assessment, or your organisation might choose to develop its own tools.

Thank you in advance for providing the requested information.

If you have any questions about this survey please contact your WA Primary Health Alliance Contract Manager.
