Pinjarra Massacre Commemoration Forges Progressive Future

Peel region community members congregated on the banks of the Murray River on Sunday 28 October for the Back to Pinjarra Day event, to commemorate the 184-year anniversary of the Pinjarra Massacre and pay respects to ancestors who have fallen.
According to Shire President Cr. David Bolt, the event created cultural awareness and assisted the spiritual healing of the Bindjareb people from the Noongar nation and the first Australians.

Aged Care: Making Sense of a Complex System

On Tuesday 9 October the WA Primary Health Alliance’s South-East metropolitan team supported the Belmont Neighbourhood Care group and Bentley Health Service to coordinate and host Aged Care: Making Sense of a Complex System in Belmont. The event focused on navigating the aged care system, optimising care for older patients and to provide an overview of National Disability Insurance Scheme (NDIS) and its impact to aged care.

One in five Australians will be affected by a mental health disorder each year. WAPHA is committed to improving the mental health and wellbeing of West Australian people.

Ground-breaking new program providing supportive intervention to vulnerable and disadvantaged people being discharged from emergency departments and justice services.
Peer workers help people in crisis to access supports such as accommodation and treatment for mental health, alcohol and other drug issues.
Targets underlying issues that contribute to repeat presentations and poor health.
The Choices discharge program is connecting vulnerable and disadvantaged people with critical social and mental health support as they are discharged from justice services and emergency departments at Royal Perth and Rockingham Hospitals.

WA Primary Health Alliance funds alcohol screening trial at Fiona Stanley Hospital and Rockingham General Hospital

In 2017/18 WAPHA provided funding to the South Metropolitan Health Service (SMHS) to determine the feasibility of alcohol screening, brief intervention and referral to GPs within emergency settings.
Patients presenting to Emergency at the Fiona Stanley and Rockingham General hospitals, who were screened and identified as high risk, were seen by the Inpatient Alcohol Team for follow up and possible referral for treatment. Those at moderate or high risk were provided a brief intervention and had their results sent to their nominated GP.

Immunisation: Education without the Pain!

WA Primary Health Alliance (WAPHA) facilitated seven lunchtime immunisation sessions in September 2018. Each educational session focused primarily on immunisation but also had updates concerning diabetes and cervical screening with over 200 practice nurses attending. PHLs attended each of the sessions and networked with the practice nurses.
The immunisation sessions were facilitated by Jane Gardiner from Metropolitan Communicable Disease Control and covered topics such as an update of the schedule changes to the National Immunisation Program, catch-up scenarios, vaccine preventative diseases, and ways to encourage better immunisation rates throughout WA.

WA Primary Health Alliance’s South-East Metropolitan team hosted its second networking opportunity for GPs in Armadale – The Who’s Who in Armadale on Tuesday 28 August which focused on building relationships and raising awareness of local services within the Armadale area.
The night started with a great turnout of over 60 attendees from GP practices, community service providers, allied health, mental health and Aboriginal services. Keeping the momentum going and the opportunities for conversation flowing the networking was focused around a game of Bingo that included key facts about the services, clinics and GPs attending the nights session.

This year our My Health Record team has been working across Western Australia to inform the community about the benefits of My Health Record and how to opt out.
Within PHN Perth South, the My Health Record team has been busy.
In Fremantle, we presented to staff at the City of Fremantle and the Fremantle Multicultural Society of WA and engaged with numerous other community groups.
