Pilbara Chronic Conditions Operations Group
Pilbara Chronic Conditions Operations Group is a subsidiary forum working to implement a work program which has been developed through periodic stakeholder workshops and the strategic directions outlined by the WA Country Primary Health Network Pilbara Regional Clinical Committee and the Pilbara Aboriginal Health Planning Forum.
The Pilbara Chronic Conditions Operations Group (PCCOG) includes individuals with a strong commitment and interest in working collaboratively to address the impact of chronic conditions on the people in the Pilbara Region and their communities.
The PCCOG endorses and recognises the following:
- The requirement to work in collaboration and form partnerships in alignment with current health reform initiatives for more effective, efficient and accessible service delivery.
- The importance of population and primary health as well as access to acute health care when needed to improve the health of the local population.
- Primary health care principles- early intervention, access, equity, reduction of disease burden, health promotion
- The need to proactively work on implementing the priority recommendations identified in the Pilbara Aboriginal Health Plan and any other relevant strategic frameworks for the prevention and management of chronic conditions.
