Thank you for participating in WA Primary Health Alliance’s (WAPHA’s) trial of POLAR. Your contribution to our understanding of the POLAR functionality as well as information communications technology (ICT), reporting and practice support resources required to offer POLAR as an alternate data extraction solution for general practices is important.
This survey should take less than 10 minutes to complete. It provides you the opportunity to tell us how you experienced the different features of POLAR, how they compared with CAT4 and your opinion on future licensing offers from WAPHA.
There are a small number of practices participating in the POLAR Trial and it is relevant for us to understand if different users or practices experience clinical audit tools differently. As such, your response to this survey is not anonymous. Responses will be held confidential to members WAPHA’s POLAR Project Team. A summary of responses from Section 1 (What do you think of POLAR) will be provided to Outcome Health. No individual respondent or practice will be identified in the sharing of this information with Outcome Health.
