WA Primary Health Alliance (WAPHA) invites Wheatbelt Community Resource Centre (CRC) managers or coordinators, to provide information which will assist us to improve health outcomes and patient experiences for those living in your community.

The survey topics include:

  1. access to Telehealth facilities
  2. culturally and linguistically diverse groups
  3. promotion of My Health Record

We are also seeking information on culturally and linguistically diverse populations in your area and your willingness to promote My Health Record.
