WA Primary Health Alliance (WAPHA) is the Primary Health Network (PHN) for Country WA and our Wheatbelt office is located in Northam. WAPHA's role includes commissioning sustainable primary care and improving health outcomes, and pharmacy plays an integral part in this. WAPHA receives funding from the Australian Government Department of Health to increase the use of digital technologies, including Telehealth and My Health Record.

By completing this survey you will assist WAPHA to determine whether Wheatbelt pharmacies have Telehealth, My Health Record and after hours capability, now and in the near future. The survey results will be used to direct our engagement with you and support you to provide better health services to our communities.
